TERMS AND CONDITIONS OF USE

It’s important that you know how we at Bronson handle information communicated on the Internet. This Terms & Conditions statement outlines our practices.  We reserve the right to stop anyone, including you, access to your Bronson MyChart any time for any reason.

Bronson MyChart - Once you request a Bronson MyChart, you will be given a one-time-only access code that you will use to start your account.

You will then create your own Bronson MyChartID, password and password hint.

DO NOT SHARE your password with anyone. Keep your ID and password safe to protect the privacy of your information. You must remember your Bronson MyChartID and password. You must also keep them confidential. From the time you receive your temporary, one-time use access code, you will have 60 days to log-on and start your account. After 60 days, your access code will expire and you will have to start all over again. PLEASE activate your account as soon as possible.

Response to Electronic Communication – We will make every effort to provide a timely response to electronic inquiries. You should allow at least three (3) business days for a response.

Emergency or urgent situations requiring immediate attention should never be sent electronically. Information you send must be complete so that we can make safe decisions about your care. If there is not enough information given, we may be unable to respond.

Web Site LinksBronson MyChart may offer links to related medical Web sites not managed by Bronson Healthcare Group. These Web site link(s) are for information only. Bronson Healthcare Group has not approved the information in/on these Web sites. Do not rely on any of the information found on any Web sites for treatment or diagnosis.

Minor Eligibility – You must be 12 or older to request a personal MyChart account.

NOTE TO PARENTS: Parents or guardians of children between the ages of 12 and 18 must work with their children and the Hospital to establish access to the child's account (see “Proxy Access").

If you are age 12-17, under Michigan law there are certain types of medical treatment that you may consent to without notifying your parents. When you reach the age of 12, automatic parental and guardian access will stop. If you are 12 or older, you must grant proxy access to your parent or guardian before they will be allowed to look at your Bronson MyChart.

Automatic proxy access will also stop when you reach the age of 18. Under Michigan law, the Hospital must consider any person over 18 to be an adult. If you are 18 years old or older, see “Proxy Access” below to establish MyChart access for anyone you want to have access to your MyChart.

Proxy Access - A “proxy” is a person you allow to look at your MyChart information. A “proxy” may also be a person who has the legal right to look at your medical information.

If you are a minor child under 12 years old (up to your 12th birthday) your parent or legal guardian is legally considered to be your proxy. A parent or legal guardian in this case has the automatic right to access and look at your records. Other individuals, like legal guardians, may request proxy access. This will be given if the requesting party can demonstrate the legal right to the information. You must be 18 years or older and be named as a proxy to look at another person’s Bronson MyChart account. You must complete the Proxy Access Form and get permission from the person whose record you want to look at.

A Bronson MyChart account will be set up for both the proxy and the account holder. If the proxy’s legal relationship with the account holder changes, the proxy must inform the office immediately.

Bronson reserves the right to revoke proxy access at any time for any reason.

E-mail Privacy - Patients who are users of Bronson MyChart will be notified via e-mail when there is new medical information entered on Bronson MyChart.

Any person with access to your e-mail will be able to see this notification. This could include your spouse, employer or anyone else who can access or see your e-mail account. No private medical information will be sent in this e-mail but the e-mail message could provide information that you may not want others to know. Please take this into account when you set up a MyChart account. When you send us an e-mail communication, it may be shared with the Bronson practice staff that assists the physician in providing your care. Only appropriate clinical staff can look at your confidential medical information on Bronson MyChart.

Security and Confidentiality – We treat the information on your Bronson MyChart the same as we treat information in your confidential hospital medical record.

Bronson Healthcare Group is committed to protecting the confidentiality of your medical information.

Firewalls, passwords, encryption, and audit trails are used to safeguard your information.

To view private information, Bronson MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape, Chrome, or Internet Explorer versions 5.0 or greater).

Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4). You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server.

This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Bronson MyChart by viewing the digital certificate. This certificate verifies the connection between the Bronson MyChart server's public key and the server's identification.

Bronson Internet Online Bill-Pay Refund Policy

In the event a customer is owed a refund due to an overpayment or credit due, once verified by the Billing Office, that refund will be tendered to the customer by their choice of either mailed check or credited to the original credit card. Requests will be handled in a timely manner. For any questions concerning a refund due or about our policy please contact Bronson’s Billing Office at 269-341-6117.

Third Party Applications

You may choose to use other services provided by third parties in conjunction with your use of MyChart. Such services may include, but are not limited to, translation services, location services, and other third-party offerings. Any links to such services are provided for your convenience only. Neither Epic nor Bronson have control over the contents of these services, and neither Epic nor Bronson accept any responsibility for them or for any loss or damage that may arise from your use of them. Such services may require you to submit your information to a third party, which may result in a copy of your information being used and/or stored by a third party. If you decide to access any such services in conjunction with your use of MyChart, you do so entirely at your own risk and subject to the terms and conditions of use for such services.
